Diabetes Navigator 

Location: Union of Ontario Indians Head Office (Nipissing First Nation) 

This is a contract position (with the possibility of an extension) to February 26, 2027, with benefits including a defined contribution pension plan starting day one and employer-paid group insurance following three months of employment, in accordance with plan terms.

Salary Range: $62,373 to $70,690 

Reporting to the Community Health Programs Manager, the Diabetes Navigator works in partnership with First Nation Diabetes staff to support Anishinabek First Nation members living with Diabetes through education, awareness, and service navigation. The role promotes healthy living practices using culturally appropriate, traditional, and innovative approaches, and assists individuals and families in connecting with available community and regional resources when appropriate. All activities are delivered in alignment with the objectives of the Aboriginal Diabetes Initiative (ADI).  

QUALIFICATIONS: 

  • Post-secondary diploma, degree, or certificate in a Health-related field; 
  • Minimum two (2) years of experience in a community-based health, wellness, education, or support role; 
  • Experience working with First Nation communities or Indigenous organizations, considered an asset; 
  • Working knowledge of Diabetes prevention, awareness, and healthy living practices, including diabetes service delivery systems;  
  • Demonstrated proficiency in Microsoft Office (Outlook, Word, Excel, PowerPoint), and relevant programs and software, with the ability to learn new technologies and tools; and 
  • Must possess a valid Ontario Driver’s License, be insurable under the organization’s vehicle insurance policy, and be willing and able to travel regularly for work-related events, meetings, and other duties as required.

  • Excellent written, verbal, and interpersonal communication skills; 
  • Strong organizational and time-management skills with the ability to manage multiple community priorities; 
  • Demonstrated ability to apply a culturally respectful, strengths-based, and trauma-informed approach; 
  • Ability to build and maintain positive relationships with community members and partners; 
  • Exhibit professional attitudes and behaviour; 
  • Good analytical and problem-solving skills; 
  • Experience in conducting workshops and training sessions online and in person; and 
  • Ability to understand and speak Anishinaabemowin, or willingness to learn. 

RESPONSIBILITIES: 

  • Develop and maintain culturally appropriate, community-based resources to support Diabetes awareness, prevention, and healthy living in First Nation communities; 
  • Organize and deliver workshops and presentations related to Diabetes awareness and prevention; 
  • Support equitable access to Diabetes education, programs, and services for individuals and families living with Diabetes; 
  • Work to increase the availability and coordination of Diabetes programs and services within Anishinabek First Nation communities; 
  • Develop and coordinate training and professional development opportunities for community-based Diabetes workers; 
  • Maintain accurate program records, statistics, and activity reports in accordance with Aboriginal Diabetes Initiative (ADI) requirements; 
  • Develop education and awareness materials, including pamphlets, newsletters, and tools; 
  • Prepare briefing notes, reports, and memorandums related to program activities; 
  • Participate in meetings, working groups, and training related to Diabetes prevention and management, as required; 
  • In collaboration with the Community Health Programs Manager, contribute to program planning and direction; and; 
  • Perform other duties as assigned.
